About Genesis Community Rehabilitation. Inc.
Genesis Community Rehabilitation. Inc. is a dedicated team of rehabilitation specialists, including occupational therapists, case managers, speech-language pathologists, social workers, and rehabilitation therapists. Our mission is to help individuals with severe injuries or acquired conditions restore function, independence, and dignity. We provide rehabilitation services in clients’ homes, schools, and workplaces.
We are committed to supporting children, youth, adults, and their families while strictly adhering to the Personal Health Information Protection Act, 2004 (PHIPA), which regulates the collection, use, and disclosure of personal health information in Ontario.

Consent

Genesis Community Rehabilitation. Inc. collects, uses, and discloses personal health information (PHI) only with your express or implied consent. You have the right to withhold or withdraw your consent at any time. In cases where PHI is used or disclosed without consent, it is done solely as permitted or required by law.

Collection of Personal Health Information

We collect PHI directly from you or your authorized representative. This may include your name, date of birth, contact details, health history, family health history, and records of care and treatment received from our professionals.

With your consent or as permitted by law, we may also gather information from other healthcare providers, insurers, or legal representatives. We only collect the information necessary to achieve the intended purpose and will not request PHI if other data suffice.

Use and Disclosure of Personal Health Information

Your PHI is shared among the Genesis Community Rehabilitation. Inc. team to ensure you receive the highest quality rehabilitation services. It may also be used or disclosed to:

We take safeguarding your PHI seriously. Measures are in place to protect your physical and electronic records from loss, theft, or unauthorized access. In the rare event of a data breach, you will be notified promptly.

You have the right to access and request corrections to your PHI records. To do so, please submit a written request to our Privacy Officer (details below).
